A semaglutide tablet is swallowed in the morning, before you eat, drink or take anything else. It has to be taken on an empty stomach, after at least 8 hours without food, with no more than half a glass of water (120 ml). For at least 30 minutes after the tablet, do not eat, drink or take any other oral medicines (1, 2). The same rules apply to semaglutide tablets for treating obesity (Wegovy) and to semaglutide tablets for type 2 diabetes (Rybelsus).
Things get more complicated if you take other medicines in the morning, especially ones that also have to be taken on an empty stomach. These include levothyroxine, a medicine used for an underactive thyroid (hypothyroidism). In a study, when it was swallowed together with a semaglutide tablet, total exposure to thyroxine (the overall amount in the blood over time) was 33% higher than after levothyroxine alone (1, 3). This is why the doctor may order a TSH test. The order of your morning medicines and the timing of tests are set by a doctor who knows all the medicines you take.

In brief
- The semaglutide tablet is taken before any other medicines, on an empty stomach, with no more than 120 ml of water. Wait at least 30 minutes before food, coffee, tea or other medicines, because any earlier they reduce the absorption of semaglutide.
- Do not take levothyroxine at the same time as semaglutide. You can take it at least 30 minutes after semaglutide, and have breakfast half an hour after that. The doctor may also advise you to take levothyroxine in the evening.
- If you take a medicine for which a small change in its blood level matters, such as acenocoumarol, warfarin, levothyroxine, lithium or phenytoin, the doctor may order more frequent tests: INR, TSH or the level of the medicine in the blood.
- In a study, the semaglutide tablet did not change the levels of the hormones in a combined contraceptive pill to a clinically significant extent. However, if you have vomiting or severe diarrhoea within 3-4 hours of taking a contraceptive pill, you usually follow the advice for a missed pill in that pill's leaflet.
How should you take a semaglutide tablet if you also take other medicines in the morning?
The semaglutide tablet is swallowed whole in the morning, before any other medicines. It has to be taken after at least 8 hours without food, with no more than half a glass of water. You can eat, drink and take other oral medicines only after at least 30 minutes (1).
The tablet must not be split, crushed or chewed, because it is not known how this affects the absorption of the medicine. Always take one tablet a day, and do not take two lower-dose tablets instead of one higher-dose tablet (1). Your other morning medicines need to be pushed back by at least half an hour.
Why is a semaglutide tablet taken on an empty stomach with no more than half a glass of water?
Semaglutide is a peptide, a molecule made of amino acids that digestive enzymes easily break down. This is why the tablet contains an added ingredient (excipient), salcaprozate sodium (SNAC), which helps the medicine to be absorbed (1, 4). Studies in humans and dogs have shown that semaglutide is absorbed in the stomach, only in the area right next to the surface of the dissolving tablet. There, the excipient protects it from being broken down (4). Even when the tablet is taken correctly, only about 1-2% of the semaglutide it contains reaches the blood. This amount varies markedly from person to person, and in the same person from day to day (1).
Food and larger amounts of fluid reduce it even further (1). In a study in healthy volunteers, people who had eaten a meal half an hour before the tablet had very little semaglutide in their blood, or none that could be measured (5). When the tablet was taken with a full glass of water (240 ml), there was less of the medicine in the blood than when it was taken with 50 ml (6). The amount of medicine absorbed with 50 ml and with 120 ml of water was similar. The longer participants waited to eat after the tablet, the more of the medicine was absorbed (5). This is why 30 minutes is the shortest gap allowed, and you can wait longer (1).
Can you drink coffee straight after a semaglutide tablet?
No. Wait at least 30 minutes after the tablet before having coffee, tea, juice or any other drink, just as with breakfast (1, 2). The tablet itself is swallowed with plain water, not with coffee or juice (1, 6).
What should you do if you eat something or take another medicine less than 30 minutes after semaglutide?
Less semaglutide will be absorbed that day, but do not take a second tablet. Take the next one as usual the following morning (1). If this happens often, you need to tell your doctor. If the medicine seems to work less well, the reason may simply be that too little of it is being absorbed, and the doctor takes this into account when assessing your treatment (1).
How do you take a semaglutide tablet alongside levothyroxine?
The semaglutide tablet and levothyroxine are taken separately. Semaglutide comes first, because there must be no food or other tablets in the stomach at that time. Levothyroxine is taken at least 30 minutes later, and you can have breakfast half an hour after it (1, 7).
This is what the morning of someone who takes both medicines might look like:
| When | What you take | What to watch out for |
|---|---|---|
| After waking up, at least 8 hours after your last meal | semaglutide tablet, no more than 120 ml of water | swallow the tablet whole, with water only (1) |
| At least 30 minutes after semaglutide | levothyroxine with a small amount of liquid, such as half a glass of water | do not eat or drink coffee between the tablets (1, 7) |
| At least 30 minutes after levothyroxine | breakfast and coffee, and your other medicines according to their own instructions | iron, calcium and antacids containing aluminium no earlier than 2 hours after levothyroxine (7) |
This plan follows the recommendations for both medicines. With this plan, there is at least an hour between waking up and breakfast. It helps to agree the exact times with your doctor, especially if you need to take more medicines in the morning.
This schedule was used in an observational study in Romania. In one of its three groups, 70 people with type 2 diabetes and hypothyroidism took a semaglutide tablet in the morning on an empty stomach and levothyroxine at least 30 minutes later. After six months, median TSH in this group (the middle value) fell from 3.1 to 2.6 mIU/l (8). However, participants were not randomly assigned to groups, and the result applies to people with both conditions. It cannot be used to predict how TSH will change in a particular person.
Can you take levothyroxine in the evening and a semaglutide tablet in the morning?
Levothyroxine can be taken in the evening if the doctor treating your thyroid decides on this. The summary of product characteristics (SmPC) for one of the levothyroxine products available in Poland (Euthyrox N) recommends taking it in the morning, half an hour before breakfast (7). The American Thyroid Association guidelines advise taking levothyroxine consistently, where possible, 60 minutes before breakfast or at bedtime, at least 3 hours after the evening meal (9).
The time of day you take levothyroxine changes the TSH result. In a Dutch study, 90 people with hypothyroidism took levothyroxine in the morning for 3 months and at bedtime for the next 3 months, or in the reverse order. When they took it at bedtime, their TSH was on average 1.25 mIU/l lower (10). So after changing the time you take it, you need a TSH test at a time set by the doctor.
Why might a doctor order a TSH test when you take semaglutide tablets?
The doctor has two reasons to check TSH. The first is the result of a study in which healthy volunteers who had been taking semaglutide tablets for several weeks swallowed a single 600 µg dose of levothyroxine together with their semaglutide tablet. Total exposure to thyroxine, meaning the overall amount in the blood over time, was 33% higher than after levothyroxine alone, although the peak level of the hormone was the same. When they swallowed levothyroxine with the excipient alone, without semaglutide, total exposure to thyroxine did not change (3). This is why, in people taking both medicines, the doctor should consider testing thyroid function (1).
The second reason is weight loss. The dose of levothyroxine depends on body weight, and even more on fat-free mass, meaning everything except fat tissue. The guidelines recommend adjusting the dose when body weight changes substantially, and testing TSH 4-6 weeks after each change in dose (9). TSH is also one of the tests done before obesity treatment, so the doctor will be able to compare the new result with the earlier one.
What signs may mean your levothyroxine dose is too high?
Do not change your levothyroxine dose on your own, either up or down. If you develop palpitations, shaky hands, sleeplessness or excessive sweating during treatment, you need to contact a doctor, because these may be symptoms of too high a dose of the hormone (7). Chest pain or a very fast, irregular heartbeat requires urgent medical help.
Biotin can interfere with TSH test results
Biotin, also known as vitamin B7, can make thyroid test results come out falsely low or falsely high. If you take it, you should tell your doctor and, before a blood sample is taken, the laboratory staff as well (7). The problem mainly concerns supplements with a high dose of biotin, and patients do not always know they are taking it (11). Biotin is a common ingredient of supplements for hair, skin and nails (12), and hair loss is one of the common side effects of semaglutide tablets for treating obesity (1). Before you reach for such a supplement, check what it contains.
How can you combine a semaglutide tablet with other medicines taken on an empty stomach in the morning?
Every oral medicine is taken at least 30 minutes after the semaglutide tablet (1). The most planning is needed for medicines that also have to be taken right after waking up, washed down with a lot of water or spaced apart from other medicines.
| Medicine | How it is taken | How to fit it around the semaglutide tablet |
|---|---|---|
| Levothyroxine | in the morning on an empty stomach, half an hour before breakfast (7) | at least 30 minutes after semaglutide, or in the evening if the doctor decides on this |
| Alendronate, a once-weekly medicine for osteoporosis | straight after getting out of bed, with a full glass of water (at least 200 ml), at least half an hour before eating, drinking or taking other medicines. You must not lie down for at least half an hour and until your first meal (13) | both medicines are taken on an empty stomach and apart from other medicines, and their SmPCs do not give a joint schedule, so the plan for the day you take alendronate is set by the doctor |
| Omeprazole, a medicine that reduces stomach acid | preferably in the morning, 30-60 minutes before the first meal (14) | at least 30 minutes after semaglutide. No clinically significant change in exposure to semaglutide was found with omeprazole (1), but it may reduce the absorption of levothyroxine (7) |
| Iron or calcium preparations, antacids containing aluminium | at least 2 hours after levothyroxine (7) | at least 30 minutes after semaglutide, and if you also take levothyroxine in the morning, not until 2 hours after it |
Alendronate and the semaglutide tablet are both taken on an empty stomach and apart from other medicines, but alendronate is taken with at least 200 ml of water and semaglutide with no more than 120 ml. The plan for the day you take alendronate is set by the doctor who prescribed it, together with the doctor managing your obesity treatment.
Do you need to wait 30 minutes after a semaglutide tablet before taking supplements?
Yes. It is best to treat them in the same way as medicines. The recommendation in the SmPC refers only to other oral medicines. However, in a study, even five placebo tablets, containing no active substance, swallowed together with semaglutide reduced the amount of the medicine absorbed by about a third (34%) (1, 3). So the mere presence of other tablets in the stomach reduces the absorption of semaglutide. This is why it is also better to take vitamins and minerals in tablet form, such as vitamin D or magnesium, after 30 minutes.
Which medicines may need more frequent monitoring when you take semaglutide tablets?
More frequent tests may be needed mainly for medicines with a narrow therapeutic index. With warfarin and acenocoumarol, more frequent INR tests are recommended after starting treatment, and with levothyroxine the doctor should consider checking thyroid function (1). With other such medicines, such as lithium, phenytoin, tacrolimus or ciclosporin, the doctor who prescribed them decides on additional tests (6).
A narrow therapeutic index means that a small change in the level of a medicine in the blood can make it stop working or start causing harm (15). Semaglutide slows down stomach emptying, so it may change the absorption of other oral medicines (1). This is why the US prescribing information for Rybelsus recommends that, with such medicines, the doctor should consider more frequent monitoring or laboratory tests (6).
| Medicine | What is known about combining it with semaglutide tablets | Recommendation |
|---|---|---|
| Acenocoumarol, warfarin (anticoagulants) | in a study, the effect of warfarin did not change, but cases of a fall in INR have been described with acenocoumarol | more frequent INR tests after starting treatment (1) |
| Levothyroxine | when both medicines were swallowed together, total exposure to thyroxine was 33% higher | consider checking thyroid function (1, 3) |
| Digoxin | a study found no clinically significant change in its level or total exposure | no separate recommendation (1). The doctor decides on monitoring, as with other medicines with a narrow index (6) |
| Lithium, phenytoin, tacrolimus, ciclosporin, theophylline | these are medicines with a narrow therapeutic index (15), but the semaglutide SmPC gives no separate data for them | the doctor who prescribed the medicine decides whether to test its level in the blood, so you need to tell them about semaglutide (6) |
| Rosuvastatin | total exposure 41% higher, considered not clinically significant | no changes needed (1) |
| Metformin, furosemide | no clinically significant changes in level or total exposure were found | no changes needed (1) |
In type 2 diabetes, one more interaction has to be taken into account. Semaglutide taken with a sulfonylurea, such as gliclazide, or with insulin increases the risk of low blood sugar (hypoglycaemia). This is why the doctor may lower the dose of these medicines at the start of treatment (1). Our article on weight gain caused by medicines explains how obesity is treated when you need to keep taking lithium or another psychiatric medicine.
Does a semaglutide tablet make the contraceptive pill less effective?
In an interaction study, the semaglutide tablet did not cause clinically significant changes in the levels of ethinylestradiol and levonorgestrel, the hormones contained in one of the combined contraceptive pills (1). Women of childbearing age should use effective contraception throughout treatment (1). Semaglutide is stopped at least 2 months before a planned pregnancy, because it stays in the body for a long time (1).
The contraceptive pill is taken every day at roughly the same time, but it does not have to be taken on an empty stomach (16). If you usually take it first thing after waking up, take it at least 30 minutes after semaglutide (1). It helps to discuss changing the time you take it with a doctor or pharmacist, because the acceptable delay depends on the type of pill. For example, with one of the pills containing ethinylestradiol and levonorgestrel, taking it less than 12 hours late does not reduce protection (16).
Nausea, vomiting and diarrhoea are very common side effects of semaglutide tablets for treating obesity, especially while the dose is being increased (1). If vomiting or severe diarrhoea occurs within 3-4 hours of taking a contraceptive pill, the pill may not have been absorbed. In that case, follow what the pill's leaflet says to do for a missed dose. If you have severe gastrointestinal symptoms, you also need an additional method of contraception (16). Our article on side effects of obesity medication explains how to ease these symptoms.
What should you discuss with your doctor before starting semaglutide tablets?
Before the consultation, it helps to prepare a full list of your medicines and supplements. For each one, write down what time you take it and whether you take it on an empty stomach. Based on this, the doctor sets the order of your morning medicines and the timing of monitoring tests. Include on the list:
- all prescription and over-the-counter medicines, including those taken for a short time, such as an antibiotic or a painkiller, with the dose and the time you take them
- medicines you take on an empty stomach, such as levothyroxine, alendronate or omeprazole
- medicines whose dose is adjusted according to blood tests: acenocoumarol or warfarin (INR), levothyroxine (TSH), lithium, phenytoin, and tacrolimus or ciclosporin after an organ transplant
- supplements, especially those containing biotin, iron or calcium
- insulin and sulfonylureas, if you have type 2 diabetes
- other obesity medicines and GLP-1 medicines, because semaglutide is not combined with other medicines from this group, and its combination with other obesity medicines has not been studied and is not recommended (1)
- the contraception you use and any plans for pregnancy
- your most recent TSH result, if you are being treated for hypothyroidism
In the Nasz Gabinet obesity treatment programme, you go through your list of medicines and your test results with the doctor during a video consultation. Using the criteria of the Polish Society for the Treatment of Obesity (PTLO), the doctor assesses whether medication is indicated and chooses a medicine from those available in Poland. In October 2026, semaglutide tablets for treating obesity were not yet available in Polish pharmacies and no date had been given for when they would go on sale in Poland (17), so for now the doctor in the programme prescribes injectable medicines. Every month, the doctor reviews the data from the app and decides on the next e-prescription on that basis. You buy the medicine at a pharmacy; it is not included in the price of the programme. Our article on eligibility for obesity medication explains what the doctor checks before issuing a prescription.
Frequently asked questions
Can you take a semaglutide tablet and levothyroxine at the same time?
No. The semaglutide tablet is taken on its own, on an empty stomach, and levothyroxine at least 30 minutes later. In a study, swallowing the two together increased exposure to thyroxine, and other tablets in the stomach reduce the absorption of semaglutide (1, 3). If you take them together by mistake once, do not take a second semaglutide tablet; go back to your usual schedule the next day. If it happens more often, you need to tell your doctor.
What should I do if I forget my semaglutide tablet in the morning and eat breakfast?
Do not take the tablet that day. Take the next one the following morning, on an empty stomach, as usual, and do not take a second tablet with it to make up for the missed dose (1). Taking the tablet after breakfast would achieve little, because after a meal semaglutide is absorbed very poorly or not at all (5).
How do you take semaglutide for type 2 diabetes (Rybelsus) with other medicines?
Rybelsus, which is licensed for treating type 2 diabetes, is taken on an empty stomach, after at least 8 hours without food, with no more than half a glass of water. After the tablet, you need to wait at least 30 minutes before eating, drinking or taking other oral medicines. Its SmPC reports the same interaction study results with levothyroxine, warfarin and contraceptive pills as the SmPC of the tablets for treating obesity (1, 2). Rybelsus is not licensed for treating obesity and has a different dosing schedule.
Do you also need to wait 30 minutes before other medicines if you use semaglutide injections?
No. The injection is given once a week at any time of day, regardless of meals (1). Even with the injections, however, take care with oral medicines that have to be absorbed quickly, and people taking warfarin or acenocoumarol need more frequent INR tests (1). The levothyroxine result comes from a study of the tablets and does not automatically apply to the injections, but the doctor should know that you take levothyroxine whichever form of semaglutide you use. Our article semaglutide tablets or injections? describes the other differences between the two forms and their availability.

Taking several medicines in the morning and want to start obesity treatment?
In the 6-month Nasz Gabinet programme for adults, during a video consultation, the doctor assesses whether you are eligible for treatment with medication, goes through the medicines you already take and issues an e-prescription. Every month, the doctor reviews your measurements from the app, and during consultations you also talk to a psychologist.
