TL;DR
Othello syndrome is a mental disorder manifested by delusional beliefs about a partner's infidelity, most commonly occurring in men suffering from alcohol addiction. Symptoms include pathological jealousy, surveillance, searching for evidence of infidelity, persecutory delusions, and physical and psychological violence. The disorder can lead to relationship breakdown, and after separation often subsides – but doesn't disappear, reactivating in a new relationship. Diagnosis is based on behavioral observation and ruling out rational grounds for suspecting infidelity. Treatment includes addiction therapy (if alcoholism is present), cognitive-behavioral psychotherapy, and – in more severe cases – pharmacotherapy and hospitalization. Othello syndrome can also occur without alcohol, for example, in people with paranoid personality, after trauma, or in the course of schizophrenia. The disease is not hereditary – it's an acquired disorder requiring long-term treatment and therapeutic support. What is Othello Syndrome?
Othello syndrome consists of persistent delusions related to partner infidelity. They appear even in situations where there are no premises indicating infidelity. A person suffering from this disorder is able to interpret any behavior and any situation as evidence of the partner's affair. Pathological jealousy and actions taken under its influence often lead to relationship breakdown. Symptoms of Othello syndrome after separation usually subside, as the sick person temporarily has no one to suspect of infidelity. However, this disorder only subsides and reactivates with the moment of entering the next relationship. Most often affected are men who are pathological alcoholics.Othello Syndrome – Symptoms
People with Othello complex initially often appear caring, attentive, and very engaged in the relationship. However, over time, the patient begins to show signs of excessive desire to control the partner. At the same time, they treat any protests as grounds for suspicion and react to them with growing anger. The main symptom of Othello syndrome is the belief in the partner's infidelity and faith that they have numerous lovers. The relationship experiences many misunderstandings and quarrels. The person affected by the disorder does not trust the partner and does not believe assurances of loyalty. They often demand admission of infidelity, and when this doesn't happen, they succumb to irrational delusions and start stalking the partner. Symptoms of Othello syndrome also include surveillance, constant searching for evidence of infidelity, persecutory delusions, and manifestations of aggression. A person suffering from delusional jealousy creates an elaborate partner surveillance system. A person with Othello syndrome treats even the most ordinary and completely innocent situations as evidence of partner infidelity. The wife's conversation with another man alone can serve as confirmation of unfaithfulness for the husband. Evidence of infidelity for them can be a different facial expression than usual, a mood change, or the purchase of new clothes. One of the symptoms of Othello syndrome is pathological jealousy.Persecutory Delusions
In a person with Othello syndrome, alongside delusions about infidelity, persecutory delusions often also occur. They begin to treat the lack of evidence of infidelity as a manifestation of the partner's cunning. It happens many times that the sick man believes that the partner and her lovers want to harm him. He accuses the woman of planning an attempt on his life.Comparison: Classic Jealousy vs. Othello Syndrome
| Criterion | Classic Jealousy | Othello Syndrome | |-----------|------------------|------------------| | Receptiveness to Arguments | Discussion possible | Arguments do not change conviction | | Evidence | Real or probable | Trivial or completely neutral events | | Behaviors | Control, but with calm periods | Constant surveillance, stalking, sometimes spy devices | | Self-awareness | "I know I'm exaggerating" | Lack of insight – complete conviction of infidelity |Manifestations of Aggression
Othello syndrome leads to aggression in the relationship. The sick person uses physical and verbal violence against the partner. Cannot regulate their emotions and control their own behaviors. Becomes dangerous both for the partner and for alleged lovers. With the development of the disorder, the patient's behaviors and thoughts become completely subordinated to delusions about infidelity. All actions they undertake aim to prove the partner's unfaithfulness. The sick person therefore begins to neglect their professional duties and abandons all other activities in favor of stalking, eavesdropping, and photographing the partner. How to Talk to a Person with Othello Syndrome?

Othello Syndrome Without Alcohol
Othello disease is most often associated with excessive alcohol consumption. It mainly affects alcoholics. However, it's worth remembering that numerous cases of Othello syndrome occurring without alcohol are also known. This disease also affects people addicted to drugs and other intoxicating substances, suffering from paranoid schizophrenia, and struggling with low self-esteem or trauma from previous relationships. Delusional jealousy is also favored by paranoid personality. Additionally, this disorder can be observed in elderly people suffering from senile psychosis caused by changes in the central nervous system.Othello Syndrome – What Does Treatment Look Like?
In a situation where Othello syndrome occurs in connection with alcohol addiction, the first stage of treatment is overcoming the addiction. Alcoholism must be overcome both on a physical and psychological level. The sick person must undergo addiction treatment and appropriate psychotherapy for this, taking the form of individual and group meetings. However, overcoming addiction alone, although extremely important, is not enough to overcome Othello syndrome. In the treatment of this disease, cognitive-behavioral therapy is primarily used. It enables the patient to understand the existing problem and move away from incorrect behavior patterns. The acute form of Othello syndrome requires hospitalization and administration of antipsychotic drugs, which often occurs without the patient's consent. After the intensity of symptoms decreases, the sick person can continue treatment on an outpatient basis. Therapy for delusional jealousy often lasts a lifetime.
