What the anti-alcohol injection is and how naltrexone works
"Anti-alcohol injection" is the everyday name for extended-release naltrexone, sold under the brand name Vivitrol. The doctor gives it in a single dose of 380 mg, deep into the gluteal muscle, and repeats it once every 4 weeks. One injection replaces a month of swallowing a tablet every day, and throughout that time the drug works quietly in the background, at a steady pace.
How naltrexone quiets the alcohol craving
Naltrexone is an opioid-receptor antagonist. It occupies these receptors and dims the reward system through which alcohol delivers relief and a sense of pleasure. The effect is felt as a weaker craving and less temptation to reach for another glass after the first one. The drug does not act on sobriety in the moment, only on the very mechanism of the loss of control. It is not addictive and does not alter consciousness.
Why the anti-alcohol injection is not the implant
It is worth drawing this distinction straight away, because in the search box the anti-alcohol injection often blurs into the implant. The Esperal implant contains disulfiram and works by aversion: after drinking it triggers a violent, dangerous reaction of the body that is meant to deter. Naltrexone stands on the other side. After it is given, alcohol causes no unpleasant effects, it simply stops being so tempting. Two different drugs, two different philosophies of treatment, and two different conversations worth having with the doctor.
Injection, implant or tablets: how the methods differ
In the pharmacological treatment of alcohol addiction in Zielona Góra, three options are used. They do not line up in a ranking from worst to best, because what works for one person will be contraindicated for another.

When the anti-alcohol injection wins on convenience
The greatest advantage of the injection is that it does not depend on goodwill on a given day. A tablet can be skipped exactly when the craving is at its strongest, whereas a drug given every 4 weeks works independently of momentary motivation. This does not mean, however, that the injection is "better" by definition. The state of health, contraindications and the goal of treatment decide the choice, which is why everything starts with a conversation with the doctor, not with a ready prescription.
The treatment path: from the first phone call to a rhythm every 4 weeks
The anti-alcohol injection is not given "on the spot". It is a short but orderly route, along which we guide the patient from start to finish, so that at no stage are you left alone with the formalities.
The four stages of the naltrexone path
In practice it looks like this. First there is contact and a qualifying visit, at which the doctor assesses whether naltrexone depot is safe for you. Then a full set of tests that confirms this assessment. Next, bringing the drug in through targeted import (import docelowy), because Vivitrol cannot be bought off the shelf at a pharmacy. Finally, the first administration and a calendar date for the next one, in 4 weeks. The two middle stages take a little time, which is why we say honestly: between qualification and the first injection, usually anywhere from a few to a dozen or so days pass.
What the patient gains from the monthly injection rhythm
The point of this path is simple: to swap thirty daily decisions about a drug for one visit a month. In Murphy's 2022 meta-analysis (the journal Addiction, 7 randomized trials) naltrexone depot was associated with on average two fewer drinking days a month and a clearer drop in heavy-drinking days when treatment ran longer than 3 months. The authors caution that the data reach only six months, and that the result is driven mainly by two large trials. That is why we treat the injection as real support that raises the odds, not a promise of abstinence that solves the problem on its own.
Qualification and tests before the anti-alcohol injection
The first stage of the path, the qualifying visit, has a single aim: to check that the injection will not harm anyone. Naltrexone depot is a prescription drug with a serious action, so it is not given "on request", but only after the history is taken and risk is ruled out.
What the doctor asks about before the naltrexone injection
The interview covers the history of drinking and of earlier attempts at treatment, the medicines you take (especially painkillers and cough remedies containing opioids), coexisting illnesses and your mental state: mood, thoughts of giving up, the risk of relapse. The most important warning: lowered tolerance to opioids
While the drug is active, the body does not respond to opioids the way it usually does. An attempt to "break through" the blockade with a larger dose risks stopping your breathing. Just as important, toward the end of the cycle and after naltrexone is stopped, opioid tolerance is lowered, so a dose that was well tolerated before can turn out to be fatal. For this reason, for at least 30 days after the last injection you must not take opioid medicines without the doctor's knowledge, and it is wise to carry information about the therapy on you in case of an accident or an operation.
When we do not give the anti-alcohol injection
The contraindications are: current use of opioids or substitution therapy, an active withdrawal syndrome, acute inflammation and severe failure of the liver, pregnancy, and an allergy to the ingredients of the drug. We take separate care when a procedure requiring strong painkillers is planned. The possible adverse effects include nausea, headache, fatigue and reactions at the injection site. At the monthly visits the doctor looks at the injection site, asks about wellbeing and mood, and if needed orders a liver check.
Targeted import (import docelowy): the lawful route to Vivitrol in Poland
Vivitrol has no Polish registration, but that does not mean it is illegal. It is brought in for a specific patient under targeted import (import docelowy), a procedure written directly into the regulations. This is a fundamental difference from the "disulfiram injections" circulating online, which are not legally available on the market in injectable form.
How we bring Vivitrol in for a patient from Zielona Góra
The doctor files the request for the drug electronically. The application is confirmed by a consultant in the relevant field of medicine, for which the regulations allow 7 days, and then the Minister of Health grants consent. Outside a hospital, a community pharmacy dispenses the drug. The request stays valid for 60 days from confirmation, and the targeted-import prescription is filled within 120 days. How much Vivitrol costs and whether it is reimbursed
The details of dosing and storage are set out in the manufacturer's product information enclosed with the package. It should be known that although filing a reimbursement application is formally possible, the AOTMiT Transparency Council found consents for the reimbursement of Vivitrol in the alcohol indication to be unjustified.
